I presented three cases which included acute pancreatitis, pyelonephritis and CHF exacerbation as well as two journal articles and 10 pharm cards. I chose those three cases because I worked closely with these patients and thought their presentation and diagnosis were interesting. I had the pleasure of meeting my site evaluator Ronald Combs PA-C who is an exceptional healthcare provider and acted more like a mentor. We went through the cases and he asked me what my thought process was in explaining my differentials. He advised me to create a more broad differential which I definitely will do in the future and also advised me to “never ever put defer” in the physical exam section. He reminded me that it is best to explain why the physical exam was not performed whether it be the patient refused or the physical exam was more focused based. Overall, I plan on taking his advise and implementing it into future H&Ps.
